The Voice for Health Homes in New York

The NY Health Home Coalition is a trade association of Health Home leaders who advocate for the best services for our members. We educate policy makers, healthcare providers, and the public about the benefits of Health Home care management. Our community comes together to share best practices that improve the quality of care management systems and services. We speak with a collective voice to influence policy makers about key issues that affect Health Homes.

What is a Health Home?

A Health Home is a network of care management providers working together to coordinate care for Medicaid members with complex healthcare needs. These members may have mental health issues, substance use problems, HIV/AIDS and/or multiple chronic conditions. Health Homes are not brick and mortar facilities.

Health Homes provide individualized, community-based care that extends beyond medical treatment to address the social determinants of health (such as housing, employment, and education).
