For over 20 years Laurie Lanphear has dedicated her career to serving the most vulnerable New Yorkers in Medicaid Care Management. In her current role as the Director of Program Services at Alliance for Positive Health, Laurie has provided oversight of Health Home Care Management services across 15 counties in Northeastern New York. In addition, Laurie has overseen all the DSRIP related activities and Value Based Payment readiness for Alliance for Positive Health.
Laurie’s expertise lies in addressing social determinants of health and health disparities for those living with HIV/AIDS, mental illness, substance use disorders, poverty, individuals that are homeless, food insecure, LGBTQ+ communities, and people of color. Laurie brings extensive experience in care management and client advocacy, strategic vision and execution, population health, program implementation, quality assurance and analytics, cultural humility and health literacy, and the ability to lead through change. As a member of both IHealth NYS and the CSP Coalition Laurie has been instrumental in advocacy with the New York State policy makers around the benefits of Health Home services.
Laurie has a Bachelor of Arts in Psychology from The College of Saint Rose. She is also a Certified Case Manager (CCM) since 2013.
